- Provide physical therapy services to patients according to a written physician's plan of care.
- Assess and evaluate therapeutic/rehabilitative/functional status, and participate in the development of the total plan of care.
- Perform home safety evaluations.
- Instruct patients and families/caregivers in the use and care of therapeutic equipment.
- Participate in case conferences.
- Identify patient and family/caregiver needs for other home health services and refer as necessary.
- Prepare and submit a clinical progress summary based on the attainment of goals as directed by organization policies.
- Provide inservice education programs for nursing organization personnel as needed.

- Prepare treatment plans and administer all physical therapy procedures for the full range of cases falling within the scope of a program for the treatment or prevention of physical disability or disease.

- Evaluate patients and interpret evaluation findings to determine the nature and extent of dysfunction.
